ATTENTION: Form 471 Filing Window Dates

USAC has announced that the Funding Year 2016 Application Filing window for the FCC Form 471 will open this Wednesday, February 3^rd at Noon, and will close on April 29^th.

The Form 471 is the next step in the filing process in securing your erate funding for FY 2016 (July 1, 2016 – June 30, 2017). In order for us to accurately complete your Form 471, we will need the following information on file:

 

– An approved Form 470: The Form 470 must be posted for at least 28 days prior to submitting a Form 471.

– Completed Client Furnished Information (CFI) Packet: Including enrollment, free & reduced lunch documentation, as well as one complete monthly invoice for each active account.

– Signed contracts/agreements for the services and/or equipment you intend to move forward with for both Category One and Category Two.
